Riverside Health Care
Effective May 27, 2008 Riverside Health Care Facilities will be implementing a new registration process at the three hospital sites across the district. Please be advised of the following IMPORTANT information:
Registrations may take longer due to the need to input all patient information e.g. address, phone number, etc. Please allow yourself at least 30-minutes prior to your appointment time. We ask for your patience during this implementation stage.
Your Ontario Health Card will be swiped at the time of registration. Please have your Health Card out and ready to be swiped.
Your card MUST be current and valid.
If your health card has expired or is declined at the time of registration you will be given information on how to get your card renewed and/or validated.
Failure to renew your card or acquire a valid card will result in you being billed for the services provided.
Please check the expiration date on your Ontario Health Card. If expired, contact the Ministry of Health at 1-800-465-5067.
If you hold a Red/White health card you may be asked for a second type of legal identification such as your driver’s license or birth certificate to ensure the correct and legal spelling of your name.
You will be asked for your secondary insurance plan information eg. Great West Life, Liberty Health, Blue Cross, 10 digit DIA Number etc.).
WSIB patients will be required to provide their claim number, employer name, address and phone number, and the type and date of injury.
Riverside is participating in a Regional Smoking Project. If you are 18 years of age or older and admitted to Emergency, Day Surgery or as an in-patient, you will be asked the following questions (participation is voluntary):
Have you smoked/used tobacco products in the last 30 days?
Have you smoked/used tobacco products in the last 7 days)
